Incident and Sentinel Event Reporting System

Tracking and reporting incidences and sentinel events is required by JCAHO and CHAP, the two primary home health accreditation organizations. Incident reports are highly valuable, necessary, and a critical indicator of the effectiveness of policies, procedures, and continuing education. A sentinel event is an unexpected occurrence involving death or serious physical or psychological injury.

Types of incidences include:

  • equipment maintenance and operational problems
  • inadequate supplies
  • verbal threats
  • physical threat or assault
  • unsafe neighborhood that was not rated appropriately
  • significant side effects associated with medication administration
  • errors in medication administration, dosage, route, frequency, use
  • patient not at home when visit is scheduled
  • direct exposure to any patient body fluid including needle-sticks
  • patient or employee injury during care
  • violation of patient confidentiality, privacy, and security
  • violation of clinical policies and procedures
  • violation of ethical behavior
  • violation of environmental health and safety
  • scheduling errors
  • First go to Agency > Agency Management > add Incident Subjects. You can start by adding the subjects listed above. Then, instruct your staff to add a new incidence report by going to Agency > Agency Management > New Incident. Generate a filtered incident report by going to Management Reports > Outcome Center > Incident Report.
